End of relationship...how to go about JSA

My partner and I have decided to go our separate ways as things haven't been great for a while. He has been receiving an increase for me as a qualified adult. I have found alternative accommodation (with family, as I currently can't afford to rent on my own) and don't know what comes next. Do I go into the SW office for my new address and make a new claim, or does he have to go to his office first to stop claiming for me?
I was looking at the forms and am confused, especially as it asks single, separated etc. It also asks if someone if receiving an increase for me. Does separated refer to a legal separation of a couple or to former cohabitants? What difference does it make? There are no maintenance issues or anything like that. I just want a clean break and to get on with my life.
 
You should call to your nearest SW office and make a claim. The office will arrange to adjust the payment currently being made to your partner
