Where are medical records kept

Not sure if this the right place for my question and if not please removed.

Got elderly relative in care home with dementia, the GP says that person records are kept in the care home and hospital, but how would the GP be able to treat his/her patient if s/he did not have access to medical records.

I am a bit surprised that the GP does not keep records also on their computer system......
 
The records are held in the nursing home as the medical staff there will need to access/update them throughout the day. A very careful note needs to be recorded of medications administered, sleep patterns and food/liquids taken.

Most residents in the homes with medical cards will be cared for by the doctor assigned to the home who is usually local. The doctor will access the records on his/her rounds in the nursing home.
