Medical card application Q

I am paye and Dh is self employed.
On the form there is a section for my income etc. Fine, all done.
Bud for my husband, its looking for his income and capital allowance and all that, but we are jointly assessed so on our returns, its joint. I cant break it down into just his.
I rang them and he hadnt a clue, gave me an email address to ask and Ive got no repsonse.
Anyone know how to get around this? Thanks.

Does your husband have an accountant?

He does. I am meeting him next week but was hoping to get the forms in the post. Dont think im going to be able to at this rate.
